Haworth Public Schools requires that no child be admitted to this school unless and until certification from a licensed physician or authorized representative of the State Department of Health is presented to the school administrators that such child has received or is in the process of receiving the immunizations required by the State Department of Public Health unless an exemption has been granted from the immunizations on medical, religious, or personal grounds.

Contact your regular doctor or the Oklahoma State Department of Health for locations to obtain immunizations your child may still need.

Immunizations Exemption

Oklahoma law allows exemptions for medical, religious or personal reasons. Students may have an exemption to one or more vaccines and receive the other vaccinations. In this case a student mat have an immunization record and an exemption certificate in file with the school.
